Enhanced Care Management (ECM)
Whole-Person, Community-Based Care for Medi-Cal Members
At Brainpower Wellness Institute, we believe that healing begins when individuals are seen, heard, and supported in all areas of life—not just in a doctor’s office. Our Enhanced Care Management (ECM) program delivers comprehensive, person-centered care coordination to Medi-Cal members with complex needs, helping them overcome barriers, access vital services, and live healthier, more stable lives.ECM is offered at no cost to eligible Medi-Cal recipients.


What Is Enhanced Care Management?
ECM is a statewide Medi-Cal benefit designed to help individuals with significant health and social needs navigate fragmented systems of care. It goes beyond basic case management—providing one-on-one support, advocacy, and connection to services across physical health, behavioral health, and social services.Our trained care team—consisting of care managers, health workers, therapists, and social service navigators—partners with you to:
- Build a customized care plan that reflects your goals and values
- Coordinate care between doctors, mental health providers, and social service agencies
- Connect you to housing, food, transportation, and benefit support
- Provide regular check-ins to support your health journey
- Advocate for you within complex system
Who Is Eligible?
You may qualify for ECM if you are enrolled in Medi-Cal and meet one or more of the following eligibility criteria established by the California Department of Health Care Services (DHCS):
- Currently unhoused or at risk of losing housing
- Living in temporary shelters, vehicles, encampments, or other unstable situations
- Diagnosed with a mental health condition or substance use disorder that impacts daily functioning
- May be engaged in behavioral health services or need connection to them
- Frequent use of emergency departments, hospital admissions, or psychiatric inpatient care
- Individuals with avoidable hospital use due to lack of care coordination
- Diagnosed with two or more chronic health conditions (e.g., diabetes, heart disease, asthma, obesity, hypertension)
- At risk for poor health outcomes without coordinated care
- Adults or youth recently released from jail, prison, or juvenile facilities
- Needing care reentry support or linkage to community resources
- Children and youth currently in foster care
- Former foster youth up to age 26, including those aging out of the system
- Individuals with high-risk pregnancies due to medical, behavioral, or social risk factors
- May need assistance with prenatal/postpartum care, housing, mental health, or substance use
- May be eligible for services that allow them to remain safely in the community rather than in long-term care or institutional settings
